To help speed up your first visit please feel free to print the new patient form for the Doctor you are seeing and have it ready on your first visit.  If your doctor is not listed forms are available at the reception desk.

 

SPMC HIPAA—Information—All patients must have a current HIPAA form at SPMC if you have not filled out a HIPAA form or your HIPAA form needs to be updated please print and bring with you on your next visit.
